Can't upgrade companies to SP4

R [Microsoft][ODBC SQL Server Driver][SQL Server]Insert Error: Column name or 
number of supplied values does not match table definition.I had a base 
installation of GP 9.0, no patches.  Decided to download SP4 and install.  
Install of the MSI went fine and upgrading the master databases worked, but 
going company by company, I get the following error:

What was the build number of your 9.0 installation prior to the upgrade? 
Please take a look at my article "Microsoft Dynamics GP upgrade path" at   
http://dynamicsgpblogster.blogspot.com/2008/10/dynamics-gp-upgrade-path.html. 
If I am not mistaken, you should have had 9.0 with SP2 at least before the 
upgrade to SP4.

I have a signed (but not WHQL-certified) driver that's giving security warnings when I try to upgrade it in Windows Vista. Can't add new users to existing companies
There are users associated with companies, but when I add a new user it comes back with the error "The user could not be added to one or more databases". When I delete a test user (that had company rights) then readd them I can checkoff companies successfully. For teh record, we change passwords on a quaterly basis, I wonder if there was a password with the SQL server that I'm missing. What about the Dynsa user? Do I need to change that password?
