CRM 4 Outlook Client Toolbar\folders not loading

I have one user that I have installed\uninstalled a number of times and 
Outlook does not connect to the server. The user has Outlook 2003 and XP SP3. 
I do not see the CRM icon in the taskbar like other computers. Thre is a CRM 
folder at the bottom of the Mail Folders with only deleted items. I have 
removed the crmaddin.dll and readded numerous times. I have run diagnostics 
and the only thing that shows up is that CRM was configured nder a different 
profile. I checked and see that there is only 1 Outlook profile. I am 
installing as the user (local admin) whose machine it is. I have installed on 
20 other machines without any issues.

Crash on 'Send"
My wife's Intel Celeron running ME started having this problem sporadically - now constantly. Outlook 2000 receives OK - everything seems fine until you hit the 'Send' button then Outlook crashes giving an error message that says 'Outlook has caused an error in <unknown> and has to shut down. Customized Toolbar RestoreState() fails if it has only 1 button
This issue happens only when the numbers of buttons in the tool bar is "1". Thats is, the user launches the customize toolbar dialog and removes all but 1 tool bar button. I have multiple Tool bars which I persist on application close and restore on application load. The SaveState() works fine as I notice the value "55 81 00 00" saved if only 1 button is left in the Tool bar. I have setup a number of user accounts and the relative mailboxes without any problem. Hello, we have one client pc with Outlook 2003 where the following problem occurs: if a mail is replied from a user created sub folder of the inbox, the reply will not appear in sent mails, but will end up in the same sub folder, sender and recipient appear as identical. Hi, trying to help a friend who migrated to hosted mail with MAPI. She is running Oultlook 2003 with latest updates. In the navigation there are personal folders and MAPI folders. Outlook freezes after sending one email after upgrading 4.0
We just upgraded from 3.0 to 4.0 this weekend. All client machines have had the desktop client upgraded and 2 have the same issue. One is XP with Outlook 2003 and the other is Vista with Outlook 2007. Open opens and the CRM addin initializes correctly.
